Stability of methylation markers in head and neck squamous cell carcinoma
BackgroundAs cancer progresses, methylation patterns change to promote the tumorigenic phenotype. However, stability of methylation markers over time and the extent that biopsy samples are representative of larger tumor specimens are unknown. This information is critical for clinical use of such biomarkers.MethodsNinetyâeight patients with tumor specimens from 2 timepoints were measured for DNA methylation in the promoter regions across 4 genes.ResultsThere were no significant differences in overall methylation of CCNA1 (cyclin A1), NDN (necdin), deleted in colorectal carcinoma (DCC), and cluster of differentiation 1a (CD1A) within paired specimens (p valuesâ=â.56, .17, .66, and .58, respectively). All genes showed strong correlations between paired specimens across time. Sociodemographic determinants of oral anticoagulant prescription in patients with atrial fibrillations: Findings from the PINNACLE registry using machine learning
Background: Current risk scores that are solely based on clinical factors have shown modest predictive ability for understanding of factors associated with gaps in real-world prescription of oral anticoagulation (OAC) in patients with atrial fibrillation (AF).Objective: In this study, we sought to identify the role of social and geographic determinants, beyond clinical factors associated with variation in OAC prescriptions using a large national registry of ambulatory patients with AF.Methods: Between January 2017 and June 2018, we identified patients with AF from the American College of Cardiology PINNACLE (Practice Innovation and Clinical Excellence) Registry. We examined associations between patient and site-of-care factors and prescription of OAC across U.S. counties. Several machine learning (ML) methods were used to identify factors associated with OAC prescription.Results: Among 864,339 patients with AF, 586,560 (68%) were prescribed OAC. County OAC prescription rates ranged from 26.8% to 93%, with higher OAC use in the Western United States. Supervised ML analysis in predicting likelihood of OAC prescriptions and identified a rank order of patient features associated with OAC prescription. In the ML models, in addition to clinical factors, medication use (aspirin, antihypertensives, antiarrhythmic agents, lipid modifying agents), and age, household income, clinic size, and U.S. region were among the most important predictors of an OAC prescription.Conclusion: In a contemporary, national cohort of patients with AF underuse of OAC remains high, with notable geographic variation. Our results demonstrated the role of several important demographic and socioeconomic factors in underutilization of OAC in patients with AF
Spectral Energy Distributions of Hard X-ray selected AGNs in the XMDS Survey
We present the spectral energy distributions (SEDs) of a hard X-ray selected
sample. The sample contains 136 sources with F(2-10 keV)>10^-14 erg/cm^2/s and
132 are AGNs. The sources are detected in a 1 square degree area of the
XMM-Newton-Medium Deep Survey where optical data from the VVDS, CFHTLS surveys,
and infrared data from the SWIRE survey are available. Based on a SED fitting
technique we derive photometric redshifts with sigma(1+z)=0.11 and 6% of
outliers and identify AGN signatures in 83% of the objects. This fraction is
higher than derived when a spectroscopic classification is available. The
remaining 17+9-6% of AGNs shows star-forming galaxy SEDs (SF class). The
sources with AGN signatures are divided in two classes, AGN1 (33+6-1%) and AGN2
(50+6-11). The AGN1 and AGN2 classes include sources whose SEDs are fitted by
type 1 and type 2 AGN templates, respectively. On average, AGN1s show soft
X-ray spectra, consistent with being unabsorbed, while AGN2s and SFs show hard
X-ray spectra, consistent with being absorbed. The analysis of the average SEDs
as a function of X-ray luminosity shows a reddening of the IR SEDs, consistent
with a decreasing contribution from the host galaxy at higher luminosities. The
AGNs in the SF classes are likely obscured in the mid-infrared, as suggested by
their low L(3-20micron)/Lcorr(0.5-10 keV) ratios. We confirm the previously
found correlation for AGNs between the radio luminosity and the X-ray and the
mid-infrared luminosities. The X-ray-radio correlation can be used to identify
heavily absorbed AGNs. However, the estimated radio fluxes for the missing AGN
population responsible for the bulk of the background at E>10 keV are too faint
to be detected even in the deepest current radio surveys.Comment: Accepted for publication in Ap
Isolated right ventricular failure in hyperthyroidism: a clinical dilemma
We present a unique case of a 42-year-old gentleman with newly diagnosed Gravesâ disease and isolated right ventricular failure. Extensive evaluation to include echocardiogram and cardiac catheterization were negative for significant pulmonary hypertension or coronary artery disease as potential etiologies. Hyperthyroid induced vasospasm is a rare but reported clinical entity that serves to be a clinical and diagnostic dilemma
Contributing to the creative economy imaginary: universities and the creative sector
© 2018, © 2018 Informa UK Limited, trading as Taylor & Francis Group. This paper explores the relationship between the creative economy and universities. As funders, educators and research bodies, universities have a complicated relationship with the creative economy. They propagate its practice, âbuying-inâ to the rhetoric and models of creative value, particularly in teaching, research and knowledge exchange. Third mission activities also play a role, seeking to affect change in the world âoutsideâ academia through collaboration, partnerships, commercialisation and social action. For arts and humanities disciplines, these practices have focused almost exclusively on the creative sector in recent years. This paper asks how the third mission has been a site where universities have modified their function in relation to the creative economy. It considers the mechanisms by which universities have been complicit in propagating the notion of the creative economy, strengthening particular constructions of the idea at the level of policy and everyday practice. It also briefly asks how a focus on alternative academic practice and institutional forms might offer possibilities for developing a more critical creative economy. The argument made is that the university sector is an important agent in the shaping and performance of the creative economy, and that we should take action if we wish to produce a more diverse, equitable space for learning, researching, and being under the auspices of âcreativityâ
Chandra study of an overdensity of X-ray sources around two distant (z~0.5) clusters
We present results from a Chandra X-ray Observatory study of the field X-ray
source populations in 4 different observations: two high-redshift (z~0.5)
clusters of galaxies 3C295 and RXJ003033.2+261819; and two non-cluster fields
with similar exposure time. Surprisingly, the 0.5-2 keV source surface
densities (~900-1200 sources deg**-2 at a flux limit of 1.5x10**-15 erg
cm**-2s**-1) measured in an ~8'x8' area surrounding each cluster exceed by a
factor of ~2 the value expected on the basis of the ROSAT and Chandra
logN-logS, with a significance of ~2 sigma each, or ~3.5 sigma when the 2
fields are combined (i.e. a probability to be a statistical fluctuation of <1%
and <0.04%, respectively). The same analysis performed on the non-cluster
fields and on the outer chips of the cluster fields does NOT show evidence of
such an excess. In both cluster fields, the summed 0.5-10 keV spectrum of the
detected objects is well fitted by a power-law with Gamma~1.7 similar to AGNs
and shows no sign of intrinsic absorption. The few (~10 out of 35) optical
identifications available to date confirm that most of them are, as expected,
AGNs but the number of redshifts available is too small to allow conclusions on
their nature. We discuss possible interpretations of the overdensity in terms
of: a statistical variation of Cosmic Background sources; a concentration of
AGNs and/or powerful starburst galaxies associated with the clusters; and g
ravitational lensing of background QSO's by the galaxy clusters. All
explanations are however difficult to reconcile with the large number of excess
sources detected. Deeper X-ray observations and more redshifts measurements are
